Evaluation Procedures
Psychological evaluation can consist of a variety of assessment measures that target various areas of functioning and development. Potential domains of assessment offered by Foundations include the following:
- Cognitive Development: Some aspects of cognitive functioning include verbal and nonverbal abilities, reasoning skills, working memory and attention, processing speed, and visual-spatial abilities. Measures that fall in this category include the Wechsler Preschool and Primary Scale of Intelligence, 4th Edition (WPPSI-IV), Wechsler Intelligence Scale for Children, 5th Edition (WISC-V), Wechsler Adult Intelligence Scale, 4th Edition (WAIS-IV), Mullen Scales of Early Learning (MSEL), Woodcock-Johnson Tests of Cognitive Abilities, 4th Edition (WJ-IV), Differential Ability Scale, 2nd Edition (DAS-2), Reynolds Intellectual Assessment Scales, 2nd Edition (RIAS-2), Comprehensive Test of Nonverbal Intelligence, 2nd Edition (CTONI-2), and Test of Nonverbal Intelligence, 4th Edition (TONI-4).
- Adaptive Functioning: Adaptive measures are designed to evaluate a client's daily functioning in a variety of domains, including daily living skills, communication, socialization, motor skills, and maladaptive behaviors. The primary measure used by Foundations is the Vineland Adaptive Behavior Scales, 3rd Edition (VABS-3).
- Executive Functioning: Assessment of executive functioning may include evaluation of sustained attention, impulsivity, hyperactivity, organization, planning, and cognitive flexibility. Evaluation of executive functioning skills can be helpful when assessing for attention difficulties, such as Attention-Deficit/Hyperactivity Disorder (ADHD). Tests of executive functioning include behavior rating scales, Tasks of Executive Control (TEC), Integrated Tests of Variables of Attention, Second Edition (IVA-2), A Developmental Neuropsychological Assessment (NEPSY), and Delis-Kaplan Executive Functioning System (D-KEFS).
- Verbal Learning and Memory: Measures of verbal learning and memory evaluate a client's ability to attend to, process, and retain verbally mediated information. These tests include the California Verbal Learning Test (CVLT-C, CVLT-3), Children's Memory Scale (CMS), Repeatable Battery for the Assessment of Neuropsychological Status Update (RBANS), and Wechsler Memory Scale, 4th Edition (WMS-IV).
- Academic Achievement: Measures in this area evaluate current academic skills based on a national normative sample and are integral in the diagnosis of learning disabilities. The primary test of achievement used by Foundations is the Woodcock-Johnson Tests of Achievement, 4th Edition (WJ-IV). Additional measures include the Gray Oral Reading Tests, 5th Edition (GORT-5), Nelson-Denny Reading Test (NDRT), Test of Written Language, 4th Edition (TOWL-4), Test of Mathematical Abilities (TOMA).
- Personality and Emotional Functioning: These measures address various aspects of personality traits and affective symptoms. These can help to identify mood and anxiety disorders as well as other potential maladaptive characteristics and strengths. Tests include behavior rating scales, the Minnesota Multiphasic Personality Inventory, 3rd Edition (MMPI-3), Minnesota Multiphasic Personality Inventory-Adolescent (MMPI-A), Millon Adolescent Clinical Inventory, 2nd Edition (MACI-2), Millon Pre-adolescent Clinical Inventory (M-PACI), Reynolds Adolescent Depression Scale, 2nd Edition (RADS-2), Reynolds Child Depression Scale, 2nd Edition (RCDS-2), Revised Children's Manifest Anxiety Scale, 2nd Edition (RCMAS-2), Multidimensional Anxiety Scale for Children, 2nd Edition (MASC-2), and Beck Youth Inventory, 2nd Edition (BYI-2).
- Social-Cognitive Functioning: Measures in this category can be used to identify strengths and weaknesses related to the ability to interact with others and understand social dynamics. These measures are critical when identifying Autism Spectrum Disorder but can also be useful in addressing a variety of other diagnostic questions. Primary measures used in this domain are the Autism Diagnostic Observation Schedule, 2nd Edition (ADOS-2) and Monteiro Interview Guidelines for Diagnosing the Autism Spectrum, 2nd Edition (MIGDAS-2).
Packages
Foundations offers two standard evaluation packages in addition to a range of other options to fit each family's or individual's needs. The standard packages include various domains of assessment listed above as appropriate for each client.
For children 5 years of age and younger, evaluations are offered at a flat fee of $4,950. A standard evaluation for a child under 5 years of age potentially includes a thorough developmental history with parents, direct observational testing, school observation (if applicable), developmental testing, adaptive testing, and structured social observational assessment. In addition to gathering history, testing, and observation, each family will meet with Drs. Schilling and/or Hess to review the results of the evaluation and report as well as thorough recommendations based on the findings.
For children 6 years of age to adults, evaluations are offered at a flat fee of $6,600. These evaluations will have more variability than the evaluations for younger children, with assessed domains depending upon the question each client hopes to answer through the evaluation. A majority of evaluations for school-aged children, adolescents, and young adults will entail assessment of cognitive abilities, executive functioning, academic achievement, and emotional functioning. A wide variety of other issues can also be addressed depending upon the needs of each particular client.
If deemed appropriate, an Autism Spectrum Disorder (ASD) diagnostic evaluation can be conducted, and the fee is $2650. Cognitive testing is offered at a fee of $800.
For those needs that do not fall within the standard evaluation packages, a fee of $275 per hour will be charged. Please contact Drs. Schilling and Hess with questions regarding packages and additional options.
What to Expect
The assessment process generally involves a developmental history interview followed by administration of one or more psychological and/or educational assessment measures. Although it is sometimes possible to complete the evaluation in one long appointment, it is common for clients to return for additional sessions to complete the assessment battery. In addition, there may be times in which Dr. Schilling or Dr. Hess believes that additional information about a client would be better gained in a natural environment and may discuss with families the possibility of doing a school or home visit. Once testing is completed, Drs. Schilling and Hess will analyze the data, and a report will be written. Clients will have the opportunity to meet again with Dr. Schilling and/or Dr. Hess to discuss the results and recommendations and receive the written report.
Fee and Payment Policy
As noted above, standard evaluations for children 5 years of age and under are offered at a flat rate of $4,950, and evaluations for children 6 years of age through adults are offered at a flat rate of $6,600. All other services are billed at an hourly rate of $275. Foundations is now happy to accept credit card payments through online invoicing serviced by PayPal. A $500 discount is offered to families paying by cash or check, for totals of $4,450 and $6,100 respectively. A deposit of 50% of the evaluation cost is due at the initial appointment, and the remainder is due on the day feedback is provided. Attendance of an IEP meeting is billed at the flat rate of $800. Please contact Foundations with any questions regarding medical insurance benefits. Each insurance provider generates unique rules and regulations regarding reimbursement, and Foundations cannot guarantee reimbursement for services. We recommend contacting your insurance provider directly prior to commencement of an evaluation to inquire if a service is covered.
Legal Testimony
For preparation for court appearances or other legal matters, an hourly rate of $500 applies, and testimony itself is billed at an hourly rate of $675. A 50% deposit for anticipated hours is due at the time of request for testimony, and the remaining balance will be billed following the court appearance.
